HOMA-IR Calculator (Insulin Resistance)
HOMA-IR estimates how hard your pancreas works to keep fasting glucose normal. It rises years before glucose or HbA1c does, which makes it one of the earliest warnings of metabolic trouble, and a useful number in PCOS.

HOMA-IR = glucose (mmol/L) × insulin (mIU/L) ÷ 22.5. QUICKI = 1 ÷ (log insulin + log glucose in mg/dL).

What HOMA-IR value means insulin resistance?
Below 1.0 is insulin sensitive. 1.0 to 1.9 is typical for healthy adults. Above 2.5 suggests insulin resistance and above 2.9 is significant. Cut-offs vary a little by population, so treat these as guides.
Does the test need to be fasting?
Yes. Both glucose and insulin must be drawn after 8 to 12 hours without food. A non-fasting insulin makes the result meaningless.
Is fasting insulin a Medicare-rebated test in Australia?
Fasting insulin attracts a Medicare rebate only in limited circumstances; many people pay privately for it. Ask your GP or the collection centre before the test.
What lowers HOMA-IR?
Resistance training, walking after meals, weight loss of 5 to 10 percent, sleep, and lower refined carbohydrate intake. Metformin and myo-inositol (in PCOS) also reduce it.
