Purpose

The thyroid panel evaluates how well your thyroid gland is functioning. It measures the hormones that regulate metabolism, energy, and many body systems. It also checks for autoimmune thyroid conditions which are common, especially in women.

When to Order

Recommended when experiencing symptoms like fatigue, weight changes, temperature sensitivity, hair loss, or mood changes. Also used to monitor thyroid medication effectiveness and screen those with family history of thyroid disease.

Preparation Instructions

No fasting is required. If on thyroid medication, take it consistently relative to testing (usually test before taking morning dose). Biotin supplements should be stopped 2-3 days before testing as they can interfere with results.

Why is TSH high when thyroid is underactive?
TSH (Thyroid Stimulating Hormone) comes from the pituitary gland and tells the thyroid to produce hormones. When thyroid output is low, the pituitary produces more TSH to try to stimulate production. High TSH = underactive thyroid (hypothyroidism).
What is the optimal TSH range?
While lab ranges often extend to 4.5 mIU/L, many thyroid specialists aim for TSH between 0.5-2.5 mIU/L for optimal symptom control, especially in patients on medication. The optimal range can be individual.
What do thyroid antibodies indicate?
Elevated TPO or thyroglobulin antibodies indicate autoimmune thyroid disease (Hashimoto's or Graves'). Even with normal thyroid function, positive antibodies increase risk of future thyroid dysfunction.
Should I test T3 and T4 if my TSH is normal?
If you have symptoms despite normal TSH, testing Free T3 and Free T4 can reveal conversion issues or subtle dysfunction. Some people have "normal" TSH but suboptimal T3/T4 levels.
